We know that our health is shaped by more than the absence of disease. Factors such as the physical, social and economic environment in which we live have a long-lasting impact. Lack of access to health care and good education also has a profound effect.

Conditions like these are known as the social determinants of health. When these conditions are optimized for all individuals living in a community, we can say we have a healthy community. Unfortunately, the conditions in Jefferson County are not optimized for everyone. That is why we’ve infused more than $1.75 million in Improving Conditions of Community Health grants to 124 organizations serving Jeffco.
